SEATTLE, WA (KCPQ/CNN) - A new autopsy report was released Wednesday on a woman shot and killed by Seattle police in June.
They said Charleena Lyles lunged at them with a kitchen knife.
The autopsy report revealed that Lyles was about 15-weeks pregnant and the officers shot her seven times.
According to the report, Lyles was not under the influence of drugs or alcohol at the time.
Family said she struggled with mental illness and that Seattle police, who had been called to her home 23 times, should have been aware of her mental instability.
An attorney said Lyles's death showed serious faults in the Seattle police department.
"What failures from an institutional standpoint occurred that placed the officers and Ms. Lyles in that unfortunate situation? How did it come to that? She was reporting a burglary. She was the victim," family attorney Travis Jameson said.
Lyles's father filed a claim for a wrongful death lawsuit last week.
An investigation is also ongoing into the use of force during Lyles confrontation with police.
