The Franklin County Health Department has begun administering the COVID-19 vaccination to healthcare providers who are in direct contact with patients and any individual age 65 and older.

Gadsden County Health Department
The Gadsden County Health Department has begun administering the COVID-19 vaccination to healthcare providers who are in direct contact with patients and any individual age 65 and older.
On Jan. 5, the health department announced that they would be administering 200 Moderna vaccines per week to residents aged 65 and older by appointment only.

Hamilton County Health Department
If you are a Hamilton County resident age 65 and over and are interested in getting a Moderna COVID-19 vaccine through the Hamilton County Health Department, you are instructed to fill out the request form here.

Jackson County Health Department
The Jackson County Health Department has begun vaccinating people 65 or older. To set up an appointment, call 850-526-2412.
Winn-Dixie
Winn-Dixie stores in Florida are now offering the COVID-19 vaccine to any individual age 65 and older.
COVID-19 vaccines are available by appointment only online.
Jefferson County Health Department
The Jefferson County Health Department is coordinating with local private health care providers to begin the administration of the vaccine to their front-line health care workers.
For persons 65 years of age and older, email the completed and signed Moderna Vaccine Screening and Consent Form to JeffersonCHD@flhealth.gov to be added to their vaccination schedule.
The completed form can also be delivered directly to DOH Jefferson.
Once received and reviewed, you will be contacted to schedule your vaccine administration appointment.
Lafayette County Health Department
The Department of Health in Lafayette County will be offering COVID-19 vaccinations to seniors age 65 and older by appointment.
Call 386-294-1321 to place your name on the list for a scheduled appointment.
Leon County Health Department
Florida's COVID-19 vaccine pre-registration system is available in Leon County. The system will notify individuals who are eligible to receive the COVID-19 vaccine when appointments are available in their area.
Appointments are no longer necessary but may be made here. Those without internet access can call: 866-200-3856. TTY: 833-476-1455.
To access the helpline for Leon County residents with questions, call 833-540-2054. Phone lines are open from 8 a.m. to 8 p.m.
If you have received the first dose and have not been given a second appointment or have forgotten the date, you can send an email to LeonCHD.Feed-back@flhealth.gov. In the email, type “second dose” in the subject line and in the body of the email, put your name, date of first dose, and brand of the vaccine.
Clients will be emailed an appointment for the second dose.

Madison County Health Department

Publix
Suwannee County residents who are 65 and older can sign up to receive the vaccine at a local Publix store.
Vaccination appointments are scheduled online only. Appointments cannot be made by calling Publix or the Publix Pharmacy.
If no appointments are available online, Publix advises checking back each day for updates.

Taylor County Health Department
The Taylor County Health Department is prioritizing persons 65 years of age and older and health care personnel with direct patient contact.
Those that have not registered with our office and meet the parameters are asked to contact the health department at (850) 584-5087 to be added to their in-house waitlist.
Additionally, in partnership with the Taylor County Board of County Commissioners and the City of Perry, the department is utilizing outside phone lines to assist in their efforts to quickly schedule appointments.
In addition to to their main phone number, if you get a call from (850) 843-3448 or (850) 843-3611, you are encouraged to answer and schedule your appointment.
Wakulla County Health Department
The statewide preregistration system to schedule COVID-19 vaccine appointments for individuals 65 and older and frontline health care workers, is now available to Wakulla County residents.
You can now pre-register for the vaccine and get notified when appointments are available in your area by visiting myvaccine.fl.gov or by calling 1-866-201-7331.
If you do not have internet access and want to register for an appointment, call1-866-201-7331.
Once appointments are available, individuals will be contacted by phone call, text, or email and will be assisted in scheduling an appointment. Because vaccine supply remains limited, appointments may not be available for several weeks.

Berrien County Health Department
As of Jan. 21, Berrien County Health Department has been contacting individuals to cancel appointments for those who have not yet received their first vaccination in the health department due to supply shortages in doses.
Individuals with appointments for their second dose will not be affected.

Brooks County Health Department

Clinch County Health Department

If you had a previously scheduled appointment to receive your first or second vaccine dose, your appointment still stands.
If you had a previously scheduled vaccination appointment at one of our vaccination sites and have now received your vaccine from another provider, please contact the health department.
If vaccine availability again becomes limited, health officials say scheduling could be paused again in the future.

While Colquitt Regional Medical Center has paused their COVID-19 vaccine drive-thru clinics for now, they do plan on resuming them once they receive additional doses.

South Georgia Medical Center
Appointment availability is subject to change based on SGMC’s available vaccine supply, which is determined by the State of Georgia.
To receive a vaccine from SGMC, you must register online for an appointment at one of their three sites. Make sure to enter as much information as possible during your registration to speed up your vaccination process.

John D. Archbold Memorial Hospital
Due to a state supply shortage of the COVID-19 vaccine, Archbold Memorial has temporarily stopped scheduling COVID-19 vaccine appointments through their vaccine call center.
They are also unable to schedule online vaccine requests at this time.
If you made an appointment to receive the vaccine, the hospital anticipates being able to honor all appointments that are currently scheduled. If you've received your first vaccine at Archbold, you will be able to receive your second dose at Archbold.
Health officials say to plan to receive your second vaccine at your scheduled appointment.
